We are now Located on the SW corner of 31st & 129th E. Ave., in the State Farm Building on the West Side of QuikTrip. Enter the State Farm side door and the suite is at the end of the hall.

You are required to give at least a 24 hour notice of cancellation OR YOU WILL BE CHARGED FOR MISSED APPOINTMENT. Therefore if you have been given a gift certificate or pre-purchased deal, it will considered used if a 24 hour notice is not received.

All returned checks will be charged a $25.00 return check fee and all monies will be due in cash or credit card. If using a credit card, all processing fees will also apply.
